Survey

Below are items I would like your response for. These items to you and your current strategies and abilities at getting tasks done. From the perspective of the course, it is an initial attempt to focus you thinking on how your work.

This survey consists of three chores. Your first chore requests that you choose your name from the list provided in the name field. The other chores are to read the statements in part I (17 items) and part II (11 items) of the survey and respond with your rating. Each of the items requests that you click on the radio button for the appropriate value. Radio buttons are used because only one can be selected at a given time. Selecting a radio button will automatically deselect any that are already selected for that item. Once you have marked each item click on the Submit button located at the bottom of the form. This form contains scripts that check for completeness. A submission is not complete, therefore not acceptable, until all items are complete. The scripts will indicate items within a section do not have responses.
